In some cases, even the \u2018man on the street\u2019 has become part of the remedy. \u00a0\u00a0<strong>Bone marrow transplant<\/strong>s are a case in point. <strong>Bone marrow transplant<\/strong>s have become the cure for a wide variety of <strong>life-threatening<\/strong> diseases ranging from many types of <strong>cancer<\/strong> to <strong>immunodeficiency syndromes<\/strong> and <strong>anemia<\/strong>s.<\/p>\n<p>WHAT IS <strong>BONE MARROW<\/strong>?<\/p>\n<p><strong>Bone marrow<\/strong> is the soft tissue inside a bone which produces the body\u2019s <strong>blood<\/strong> cells (red cells, white cells and platelets).<\/p>\n<p>WHAT ARE <strong>STEM CELLS<\/strong>?<\/p>\n<p><strong>Stem cells<\/strong> are immature blood cells produced in the <strong>bone marrow<\/strong>. If the <strong>bone marrow<\/strong> is damaged, the new cells produced will also be damaged. A replacement of undamaged <strong>cells<\/strong> will enable the body to produce healthy <strong>blood<\/strong> cells, thus creating the cure.<\/p>\n<p>WHAT DISEASES HAVE BEEN FOUND TO BENEFIT FROM A <strong>BONE MARROW TRANSPLANT<\/strong>?<\/p>\n<p>AML, ALL, CLL, CML, Juvenile myelomonocytic leukemia, Hodgkin lymphoma, non-Hodgkin lymphoma, severe aplastic anemia, Fanconi anemia, Paroxysmal nocturnal hemoglobinuria, Pure red cell aplasia, Amegakaryocytosis\/congenital thrombocytopenia, SCID, Beta thalassemia major, Sickle Cell Disease, Krabbe disease, Hurler syndrome, MLD, ALD<\/p>\n<p>EXACTLY WHAT IS TRANSPLANTED?<\/p>\n<p>Three sources of cells used for transplants are:<\/p>\n<ul>\n<li>Bone marrow (BM)<\/li>\n<li>Stem cells from peripheral blood (PBSC)<\/li>\n<li>Blood collected from the newborn\u2019s umbilical cord after its birth (CB)<\/li>\n<\/ul>\n<p>The doctor doing the transplant chooses the source most appropriate for the patient, taking into consideration the needs and preferences of the donor.<\/p>\n<p>WHAT IS THE DIFFERENCE BETWEEN AUTOLOGOUS AND ALLOGENEIC STEM CELL TRANSPLANTS?<\/p>\n<p>An autologous transplant involves only the patient\u2019s own stem cells which are then transplanted back into the patient several days later after high doses of chemo and sometimes radiation. An allogeneic stem cell transplant is done using cells from another donor.<\/p>\n<figure id=\"attachment_3989\" aria-describedby=\"caption-attachment-3989\" style=\"width: 300px\" class=\"wp-caption alignleft\"><a href=\"http:\/\/ezermizion.org\/blog\/wp-content\/uploads\/2017\/04\/DNA.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-3989 size-medium\" src=\"http:\/\/ezermizion.org\/blog\/wp-content\/uploads\/2017\/04\/DNA-300x216.jpg\" alt=\"DNA\" width=\"300\" height=\"216\" srcset=\"https:\/\/ezermizion.org\/blog\/wp-content\/uploads\/2017\/04\/DNA-300x216.jpg 300w, https:\/\/ezermizion.org\/blog\/wp-content\/uploads\/2017\/04\/DNA.jpg 546w\" sizes=\"auto, (max-width: 300px) 100vw, 300px\" \/><\/a><figcaption id=\"caption-attachment-3989\" class=\"wp-caption-text\">DNA<\/figcaption><\/figure>\n<p>WHY CAN\u2019T ANY KINDLY PERSON DONATE HIS <strong>MARROW<\/strong>?<\/p>\n<p>Human leukocyte antigens (HLA) are proteins that are present in most body cells. These antigens help identify tissue types. The immune system utilizes HLA antigens in order to identify the cells that belong in the body and the cells that do not belong in the body. If the system recognizes cells very different from itself, it will reject them.<\/p>\n<p>In one\u2019s tissue-typing system, there are two groups of antigens. One group is inherited from the \u00a0mother and the other group from the father.<\/p>\n<p>HLA proteins are important in determining the compatibility of donors and patients for a stem cell transplant. In order to match tissue types for a transplant, the compatibility of ten of the donor and patient antigens are checked (generally, A, DR, C, B, and DQ).<\/p>\n<p>When the transplant center, to which the blood samples were sent, examine the level of compatibility, they check the genetic similarity of the patient\u2019s and the donor\u2019s tissue types. Usually, a compatibility of at least 8 out of 10 antigens is necessary in order to approve a donor for a transplant.<\/p>\n<p>WHY IS GENETICS A SUCH A MAJOR FACTOR?<\/p>\n<p>Strict matching is necessary since the key to a successful replacement is <strong>genetics<\/strong>. If the donor matches the recipient, the transplant is not likely to be rejected and the chances of success are excellent. A sibling is a good choice.<\/p>\n<figure id=\"attachment_107\" aria-describedby=\"caption-attachment-107\" style=\"width: 161px\" class=\"wp-caption alignleft\"><a href=\"http:\/\/ezermizion.org\/blog\/wp-content\/uploads\/2012\/11\/pr-bmr-cell-bag.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-107 size-full\" src=\"http:\/\/ezermizion.org\/blog\/wp-content\/uploads\/2012\/11\/pr-bmr-cell-bag.jpg\" alt=\"pr bmr cell bag\" width=\"161\" height=\"116\" \/><\/a><figcaption id=\"caption-attachment-107\" class=\"wp-caption-text\">Stem cells ready for transplant<\/figcaption><\/figure>\n<p>WHAT IF THERE IS NO SIBLING AVAILABLE OR THE SIBLINGS\u2019 <strong>DNA<\/strong> DO NOT MATCH?<\/p>\n<p>A request is then sent by the<strong> oncology<\/strong> clinic to a universal registry which will search its database for a compatible potential donor. A match may be found among the patient\u2019s neighbors or it may be a resident of a country across the ocean. The potential donor is usually thrilled to become a crucial part of a <a href=\"http:\/\/ezermizion.org\/blog\/watching-the-grandchildren-grow-up-together\/\">life-saving medical miracle<\/a>.<\/p>\n<p>ARE ALL REGISTRIES THE SAME?<\/p>\n<p>If the patient is a member of a minority <strong>ethnic<\/strong> group and a <strong>Registry<\/strong> which focuses on that <strong>ethnic<\/strong> group exists, a request is sent to that <strong>Registry<\/strong> thus greatly increasing the chances of finding a match.<\/p>\n<p>HOW DOES THE REGISTRY DETERMINE <strong>DNA<\/strong>?<\/p>\n<figure id=\"attachment_751\" aria-describedby=\"caption-attachment-751\" style=\"width: 200px\" class=\"wp-caption alignleft\"><a href=\"http:\/\/ezermizion.org\/blog\/wp-content\/uploads\/2013\/08\/pr-bmr-cheek-swab-1545_ne_photo_stories2_12971.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-751 size-full\" src=\"http:\/\/ezermizion.org\/blog\/wp-content\/uploads\/2013\/08\/pr-bmr-cheek-swab-1545_ne_photo_stories2_12971.jpg\" alt=\"pr bmr cheek swab 1545_ne_photo_stories2_12971\" width=\"200\" height=\"252\" \/><\/a><figcaption id=\"caption-attachment-751\" class=\"wp-caption-text\">Genetic testing via cheek swab<\/figcaption><\/figure>\n<p>The new registrant <strong>swab<\/strong>s the inside of his cheek with a Q-tip. The saliva is then analyzed for its genetic components. <strong>DNA<\/strong> testing is costly ($50 per test) which is why a registry must put a cap on the amount of registrants it can accept. When <a href=\"https:\/\/ezermizion.org\/donate-to-ezer-mizion.html#tabs-2\" target=\"_blank\">donations <\/a>are made by the public,\u00a0the <a href=\"http:\/\/ezermizion.org\/blog\/because-of-you-43\/\">increased funding <\/a>enables the registry to grow, thus boosting the chances of a positive response to each search request.<\/p>\n<p>WHAT IS THE DIFFERENCE BETWEEN <strong>A BONE MARROW TRANSPLANT<\/strong> AND A <strong>STEM CELL TRANSPLANT<\/strong>?<\/p>\n<p>Both are the same as far as results are concerned. However, the method of harvesting the <strong>stem cells<\/strong> differs. Not that long ago, <strong>stem cells<\/strong> were obtained from <strong>bone marrow<\/strong> taken from the donor. This procedure was painful for the donor and often discouraged the donation. One cannot imagine the devastation by the patient and his family when a perfectly matching donor refuses to go ahead.<\/p>\n<p>WHAT IS THE PROCESS OF STEM CELL DONATION? Medication is given to the donor beforehand to increase the <strong>stem cells<\/strong> in his circulatory system. <strong>Blood<\/strong> is then taken much the same as when donating a pint of <strong>blood<\/strong>. The <strong>stem cells<\/strong> are extracted from the <strong>blood<\/strong> which is then given back to the donor. The process continues for hours but the donor is kept comfortable during the whole procedure.<\/p>\n<p>In some cases, minor discomfort such as muscle aches are felt but these soon disappear. The tremendous emotional satisfaction as he watches that little \u2018bag of life\u2019 taken to his soon-to-be blood brother will eradicate any lingering pains.<\/p>\n<p style=\"text-align:
